Medical & Disclaimer Form (Mandatory Documents)

An Indemnity Bond and Self-Declaration Medical Form are must-have documents on the trek. We legally require the trekkers to fill out these forms and submit them to us at the time of the trek. Failing to do so may result in the cancellation of your trip. Please read the information below carefully.

  • If you fail to provide these documents or if we find any discrepancies in the documents provided regarding their correctness, we reserve the right to terminate your trek with immediate action, and in this case, cancellation will be according to the cancellation policy. Self-Declaration Medical Form

The Self-Declaration Medical Form is an important document for any trekker participating in short (2-3 days) treks. It will help us assess your medical history, current medications, and fitness level.

The Himalayan treks are associated with certain risks. The decreased oxygen levels, lower air pressure, and unpredictable weather conditions can trigger Altitude Sickness (Acute Mountain Sickness - AMS), causing symptoms like headaches, nausea, dizziness, and shortness of breath. There may be other complications such as injuries, dehydration, hypothermia, or frostbite.

The information filled in this medical form will enable us to identify potential risks, plan for medical emergencies, and provide adequate care if complications arise. Filling the form also makes sure that you are physically prepared for the demands of the trek.

  • Nag Tibba Weekend Trek (2 days)
  • Kareri Lake Trek (3 days)
  • Binsar Weekend Trek (2 days)
  • Deoban Weekend Trek (2 days)
  • Chopta Chandrashila 3 Day Trek

Note: Participants suffering from asthma, blood pressure issues, heart conditions, diabetes, or any chronic disease that can be a deterrent on the trek cannot participate in the trek.

Indemnity Bond Requirement for Trekking

Trekking in the remote Himalayas involves risks, and TTH requires all participants to sign an indemnity bond confirming their voluntary participation and awareness of these risks. For trekkers under 18, parental consent is mandatory, ensuring parents acknowledge the dangers, including weather changes, natural calamities, and falls.

While we take all necessary safety measures, certain risks remain inherent in the wilderness.
